Step # 1 - Enter your Name

On the first line that says "Name" enter your name if you are an individual. If you are a business then enter the business name on the second line that says "Business name".

Step # 2 - Enter you address

On the line that says "Address" put your street address and on the line below that line write in your City then State and ZIP code.

Step # 3 - Fill in Part I

Enter your social security number or TIN in the box that says "Social security number" if you are an individual. If you are a business enter your employer identification number. If you do not have an employer identification number and you are a business you should click on employer identification number application.

If you are subject to backup witholding then cross out line #2 under Part II of the form. Then sign and date the form.

Congratulations you have done something in a minute or two that could have cost you at least an hour and tons of stress if you would have read the IRS instructions.
